| Individuals interested in participating in the Educational Talent Search Program must complete a mandatory Enrollment Application. Each part of the application must be completed, and the application must be signed and dated by appropriate persons. |

| The application should include the following documents: |

- Personal information
- Eligibility/income information
- School release and signature of commitment and certification
- Student autobiography
- Teacher recommendation
|
| The ETS staff will select eligible individuals with academic potential for post-secondary education and will assist them in preparing for and enrolling in post-secondary education with adequate financial aid. |
